Our Outreach team is here to help patients access resources to promote health and wellness that they may not know about or have access to, like SNAP. The Supplemental Nutrition Assistance Program offers nutrition help for low-income individuals and families going through difficult financial times. The goal is to help alleviate food insecurity. Certain requirements like resources and income limits determine eligibility for SNAP assistance, previously food stamps. You must apply in the state where you live. Use the prescreening tool to see if you might qualify. It only takes about 3 minutes. If you qualify, you must still apply for SNAP assistance benefits.
If the pre-screening indicated that you might qualify for SNAP assistance, contact Bluestem health for more information 402-476-1455. Or you can apply online at DHHS Access Nebraska.

• Fruits and vegetables
• Meats, fish and poultry
• Dairy products
• Seeds and plants which produce food for the household to eat
*In some areas, restaurants can be authorized to accept SNAP benefits from qualified homeless, elderly, or disabled people in exchange for low-cost meals.
• Beer, wine, liquor, cigarettes or tobacco
• Any nonfood items
• Pet foods
• Soaps, paper products
• Household supplies
• Vitamins and medicines
• Food that will be eaten in the store
• Hot foods
