Your donation supports Bluestem Health programs that address a few of the biggest needs in our city: dental care for patients on Medicaid and diabetes education for our patients with prediabetes and diabetes.  Many of the patients receiving these services have extremely limited, or otherwise cost-prohibitive, access to the care they need.   • Our no-cost Diabetes Education programs help patients gain the necessary skills and confidence to manage their condition successfully.

At Bluestem Health, we want our patients to have access to their personal health information when it is convenient for them. Our patient portal is a secure online website that gives our patients convenient, 24-hour access to personal health information from anywhere with an Internet connection.

About the Patient Portal

Using a secure username and password, you can view your health information, such as:

  • Recent Doctor Visits
  • Medications
  • Immunizations
  • Allergies
  • Lab Results

You can also perform tasks, such as:

  • Securely message your doctor
  • Request prescription refills
  • View educational materials
  • Communicate with our team

By using Bluestem Health’s patient portal, you can accomplish tasks that would normally be done through a phone call, or possibly several phone calls, between you and your provider. Having the ability to request appointments, referrals, and prescription refills directly from the portal when it is convenient for you saves both you and your provider time.

Improving Patient-Physician Relationships

Patient portals allow you to have 24-hour access to connect with your provider for reviewing patient health information, asking any questions you may have, and staying connected between times when you don’t have an appointment.
