• Access Enhancer: This award celebrates health centers that grow their patient base and expand access to vital services—like mental health care, substance use treatment, vision, dental, and support services—by at least 5%.
  
• Health Disparities Reducer: We earned this recognition for making real progress in reducing health disparities. That means either meeting national goals in areas like low birth weight, high blood pressure, or diabetes within a racial or ethnic minority group—or showing a 10% improvement in those areas.
  
• Addressing Social Risk Factors: This recognition highlights our commitment to looking beyond clinical care. It honors health centers that screen patients for social challenges, like housing, food insecurity, and transportation—and connect them with the support they need.
  
• Advancing HIT for Quality: We were also recognized for using health information technology in smart, patient-focused ways, such as improving telehealth, boosting patient engagement, making data more shareable between systems, and tracking the social factors that impact health.
